Allendale Dental’s representatives are not responsible for your insurance carrier’s determinations. In-network insurance carriers will finalize your balance according to your policy with them. You can expect an estimated patient responsibility to be collected at the time of your visit. You may receive a final request for additional payment weeks after your visit IF your insurance carrier determines you have underpaid at the time of your appointment. Your final balance will be provided by your carrier on the explanation of benefit (“eob”). Please note, if you are covered by more than one insurance plan, then coordination of benefits will require more time to process your claim and derive your final balance. Lastly, it is your responsibility to ensure you have active coverage at the time of each visit.
Allendale Dental’s billing personnel have no part in your insurance carrier’s determination other than to relay information between all parties.

Patients can use the Citi Health Card® for expenses not covered by dental insurance (including cosmetic and elective dental work). In addition, the patient credit line can be used for additional dental procedures and/or family members.You can view, manage and pay your account online, plus a separate credit line for dental health care expenses will help keep other credit available for personal use. Fees and Co-Payments
When applicable, your contracted insurance rates govern your fees, therefore co-payments and estimated responsibilities are due at the time each service is rendered unless other arrangements have been made in advance with our office manager who can be reached by phone or by email: of****@*************al.net .
Statements, Insurance, and Forms
Your paperless claim will be processed efficiently within days to weeks following your visit. Please call our office if your billing statement does not reflect your insurance’s payment within that time frame. Any balance remaining after your insurance processes your claim is your final responsibility. Prompt remittance is expected.
Flexible Financing Available
Allendale is pleased to offer no-interest financing for up to 6 months through CareCredit. This option makes it easier to manage the cost of your care with convenient monthly payments.
